RCPsych responds to 2025 competition ratios

Dr Nermeen Ahmed and Professor Subodh Dave have provided a comment on the 2025 competition ratios for core psychiatry training applications which were published last week.

“罢丑别 competition ratios for core psychiatry training applications have been published, and it is positive to see such strong interest in psychiatry, and ultimately in providing the right care and treatment for people living with a mental illness.

“However, we recognise that the recruitment system is not keeping pace with the growing need for psychiatrists, nor with the increasing number of doctors who wish to pursue a career in psychiatry. We are actively working, within the scope of our influence, to address these challenges.

“We understand that this year’s unprecedented for Core Psychiatry Training have created uncertainty. Psychiatric resident doctor colleagues, as well as foundation doctors and medical students working towards a career in psychiatry, have rightly raised their concerns.

“We are aware that there are particular concerns on two separate matters:

  1. how we are choosing the best doctors to train in Psychiatry and whether the current method, the – the selection tool for Core Psychiatry recruitment and other medical specialty recruitment managed by NHS 萝莉视频), is the best method for doing so, and
  2. concerns that UK Graduates (UKGs) no longer have clear postgraduate training progression pathways available to them as the Health and Care Worker Visa enables International Medical Graduates to work in the UK more easily and therefore doctors who may have greater experience are competing for the limited number of UK training posts.

“We have discussed this in detail at appropriate College committees, including with our Psychiatric Resident Doctors’ Committee (PRDC), the Education and Training Committee, the Dean’s Strategy Meetings and also externally at the Academy of Medical Royal Colleges.

“We have also raised these issues directly with key decision makers. In particular, the National Psychiatry Recruitment Board, NHS 萝莉视频’s National Medical Director, Professor Steve Powis, former Chief Workforce, Training and Education Officer for NHS 萝莉视频, Dr Navina Evans, and her deputy, Dr Sheona MacLeod. Furthermore, we have raised these issues with the Chief Medical Officer, Professor Chris Whitty.

“Our position is quite clear:

“We must review the way we recruit psychiatrists. With respect to the recruitment method, the College has set up a Task and Finish Group, which includes members of the PRDC, experienced College educationalists and members of the National Recruitment Board, to review both the selection methodology and also the operational viability of any proposed changes to Core Psychiatry recruitment, given the escalating competition ratios.

  • RCPsych Associate Dean for Choose Psychiatry
  • RCPsych Associate Dean for Resident Doctors' Support

  • RCPsych Specialist Advisor for Less Than Full Time Training
  • RCPsych Chief Examiner
  • RCPsych Psychiatric Resident Doctors’ Committee Representatives x2 (Chair plus one other)
  • RCPsych Child & Adolescent Run-Through Representative
  • RCPsych Intellectual Disability Run-Through Representative
  • NHSE WTE National Mental Health Clinical Advisor
  • National Psychiatry Recruitment Board (NPRB) Chair
  • NPRB Heads of School Representatives, ensuring at least one from each of the four nations

“Summaries of the meetings will be shared with the PRDC, and further engagement will be carried out through our usual stakeholder channels, including but not limited to the Heads of Schools of Psychiatry, Faculties, Divisions and Devolved Nations. The final proposed recommendations will be presented to the College’s Education and Training Committee, Council and the Board of Trustees, to ensure wide engagement and dissemination.

“It should be noted that the College’s role in National Psychiatry Recruitment is in an advisory capacity. Whilst we have psychiatrists at all levels of their career involved in their various, valued and experienced educational roles across the College, the final decision for any changes to the National Recruitment methodology and processes will be undertaken by NHS 萝莉视频’s Medical and Dental Recruitment and Selection Board (MDRS).

“We have been urging NHS 萝莉视频’s Workforce, Training and Education Directorate, who are responsible for the MDRS, to look into this matter for some time. It is vital that the four nations in the UK have a secure workforce pipeline. It is with this in mind that we have advocated for doubling the number of medical school places; we have asked for and secured an increase in the number of training places at core and higher levels; and have an International Strategy that makes clear the importance of ethical international recruitment.

“We have consistently highlighted the need for a clear and fair policy that enables the recruitment of International Medical Graduates (IMGs) in areas facing workforce shortages, in line with approaches used in other sectors under the Highly Skilled Migrants scheme. We have been informed that this is being looked into, and we will continue to raise it at the highest levels to ensure progress is made.

“We are delighted to see the increased numbers of UK medical students and foundation doctors choosing Psychiatry for their career and will continue to work with the government in helping to find a solution across all medical specialties to address the issue of prioritising UKGs. This is a decision for the government and is in line with the stated aim to reduce reliance on an international workforce. We have been reassured that around three-quarters of core training posts for August 2025 were offered to UKGs.

“It is also important and fair that IMGs seeking employment opportunities in the UK are given clear and transparent information and not given misleading information or false hope of secure employment in the UK. This is particularly important when employers offer clinical attachment experiences to IMGs.”
